چکیده

Following the recent publication of the PRAMI trial, the appearance of the article entitled ‘‘Multivessel approach in ST-elevation myocardial infarction: Impact on in-hospital morbidity and mortality’’ by Santos et al. in this issue of the Journal could not be more timely. The study analyzed a population of patients with ST-elevation myocardial infarction (STEMI) included in the Portuguese Registry of Acute Coronary Syndromes who underwent primary percutaneous coronary angioplasty (PCI), comparing patients treated by PCI of the culprit vessel only (n=180) with those who underwent PCI of vessels other than the infarct-related artery (n=77). Patients undergoing multivessel PCI were more often medicated with glycoprotein IIb/IIIa inhibitors (73.7% vs. 38.8%) and had more stents implanted (1.4 vs. 1.1 stents per lesion), more of which were drug-eluting stents (73.7% vs. 40.8%). In-hospital mortality and the incidence of complications (defined as major bleeding, need for transfusion, invasive ventilation, heart failure and reinfarction) were lower in the multivessel PCI group (2.6% vs. 7.8% and 23.4% vs. 32.8%, respectively). None of these differences reached statistical significance, probably due to the small sample size. Multivariate analysis identified

Culprit Vessel-Only vs. Staged Multivessel Percutaneous Coronary Intervention Strategies in Patients With Multivessel Coronary Artery Disease Undergoing Primary Percutaneous Coronary Intervention for ST-Segment Elevation Myocardial Infarction.

BACKGROUND We assessed the current status of treatment strategy in ST-segment elevation myocardial infarction (STEMI) with multivessel disease (MVD) in real world practice, focusing on the benefit of staged percutaneous coronary intervention (PCI). METHODSANDRESULTS From the CREDO-Kyoto AMI Registry, 2,010 STEMI patients with MVD undergoing primary PCI were analyzed. Only 96 patients (4.8%) r...

Six month results of randomized clinical trial: Multivessel stenting versus staged revascularization for ST-elevation myocardial infarction patients with second generation drug eluting stents

Background: There are no randomized trials described outcomes of multivessel percutaneous coronary interventions (PCI) (in primary and staged revascularization) with second generation drug eluting stents (DES) in patients with ST-elevation myocardial infarction (STEMI). We are presenting preliminary results of randomized trial (NCT01781715). Methods: Six-month outcomes of 89 consecutive patient...

Current Status of Coronary Intervention in Patients with ST-Segment Elevation Myocardial Infarction and Multivessel Coronary Artery Disease

Primary percutaneous coronary intervention (PCI) is a standard interventional treatment modality for ST-segment elevation myocardial infarction (STEMI). Diagnostic coronary angiogram during PCI reveals multivessel coronary artery disease in about half of patients with STEMI, and it is difficult to make decision on the extent of intervention in these patients. Although revascularization for the ...

Multivessel revascularisation versus infarct-related artery only revascularisation during the index primary PCI in STEMI patients with multivessel disease: a meta-analysis

BACKGROUND There are controversial data regarding infarct-related artery only (IRA-PCI) revascularisation versus multivessel revascularisation (MV-PCI) in ST-elevation myocardial infarction (STEMI) patients with multivessel disease undergoing primary percutaneous coronary intervention (PCI). We performed a meta-analysis comparing outcome in same stage MV-PCI versus IRA-PCI in STEMI patients wit...

Culprit lesion-only versus complete revascularization in patients with STEMI: Lessons learned from PRAMI, CvLPRIT, and DANAMI-3 PRIMULTI

BACKGROUND Primary percutaneous coronary intervention (PCI) is the current standard of care for ST-elevation myocardial infarction (STEMI). About 40–50% of patients presenting with STEMI have multivessel disease (MVD). Compared to patients with single vessel disease, patients with STEMI and MVD have higher mortality rates and a greater incidence of non-fatal re-infarction. The underlying mechan...
